Tipplasty Explained

Tipplasty, as mentioned above, is a surgical procedure focused on reshaping the lower part of the nose, particularly the “tip.” To understand this, let’s break down the structure of the nose into three main parts:

  1. Upper Area: The part of the nose closest to the skull and eyes is made of bone.
  2. Middle Area: This section is made of cartilage.
  3. Lower Area (Tip): The lower third of the nose, where the nostrils and tip are located.

In tipplasty, the focus is on the lower area, including the nostrils. The nostrils are separated by a central structure called the columella, and the outer edges of the nostril openings are called the “alar base.” Often, adjustments are made here to narrow the nostrils or shape the alar base for a more refined look.

The tip itself is at the end of the nose. Surgeons look at the tip’s width, shape, and how defined or rounded it appears. Ideally, the tip should have a defining point that sits slightly above the nose bridge (dorsum).

The angle between the upper lip and the tissue that separates the nostrils (columella) is also assessed. For women, a desirable angle is 90 degrees or more, though this varies depending on individual features and ethnic backgrounds.

The sketches below are examples of how we plan these changes to create a specific approach for each patient. There are a large number of maneuvers that can be done in order to correct a tip deformity. I have illustrated below some of the most frequent ones. They are two series of sketches. The first series relates to what can be done to correct the tip itself, and the other set of sketches illustrates what can be done to the base of the nostrils.

In the first set, the cartilage is green. The first image shows how we can sharpen the angle of the cartilage in order to sharpen the tip.

In the third set of images the orange refers to the cartilage graft that can be used either as two separate pieces or as one whole piece, there are a multitude of cartilage designs that can be used for that purpose I did not show. The last image on the bottom shows a piece of cartilage that is removed in order to narrow the tip in the front-to-back direction.

The next set of images are specifically designed for the base of the nostrils. In the first set, you can see two types of excision of tissue that do not involve the floor of the nostril. One is to narrow the bulge and the other one to thin the wall of the nostrils.

The second set of images refers to the excision of the wall of the nostril as well as the floor, and it is combined on the right with the excision of a little piece of skin to thin the wall of the nose.

In the last set of images, the base of the nose is narrowed without resection of the outside of the nose. You can see a suture in blue from one nostril to the other in order to maintain the narrowing as the tissues are healing and the patient is talking, smiling, etc.
