Upper Eyelid Surgery

Upper eyelid surgery is a corrective procedure that can enhance one’s life. After all, our eyes allow us to look into the world. Interestingly, they also allow others to read into our emotions and our physical well-being. Sadness and joy are easily visible in the expression of the eyes. Conversely, tiredness and aging are easily visible in our eyelids, as well.

What our patients tell us they want to see in their eyelids:

  • Smooth skin – free of any “crinkles”
  • Eyes wide open
  • No fat bulging in the upper or lower eyelids
  • A platform that allows their makeup to highlight the shape and color of their eyes

There are multiple variables that affect the perception of the size of the eyes:

  • The shape of the bony structure called the 
  • The strength and the attachment of the muscles that pull and open the eyelids: the levator muscles.
  • Whether there is excess skin.

Factors:

When looking in the mirror, patients frequently miss the asymmetry of their eyes. One eye is not always identical in shape to the other. Also frequently overlooked is the position of the eyebrows. An eyebrow that is lower on one side creates more skin excess on that side compared to the other.

All of these factors need to be evaluated before surgery, particularly on the day of surgery when the markings are made. The marking step is probably the most important step of the procedure and it needs to be very meticulous.

Procedure:

Once the skin incision is made in the upper eyelid, the fat is addressed. Years ago plastic surgeons would resect (trim away) the eyelid fat from the center of the lid.

Nowadays, in order to avoid a gaunt look, resection is done at the eyelid towards the nose.

Post-Op & Healing:

Fortunately the eyelid scars are very forgiving. Due to their location in the eyelid crease, they are not noticeable and not visible when the eyes are open. The sutures are removed at 5-7 days. A patient rarely takes pain medication past the first day.

Follow up appointments are made at 2 days, 5 days, 6 weeks, and 6 months. All of these are complimentary and included in the procedure cost.

Care and maintenance is fairly simple, as well. You can apply makeup freely after the sutures are removed. Showering is feasible right away, but withhold swimming for 3 weeks after surgery.

Nose Surgery

One of the most difficult areas to treat is the neck and jowl (lower cheeks). Fortunately, now there is a non-surgical method to treat this area. No scalpel, no surgery and no stitches are involved. Thermi delivers radio frequency for skin tightening. Treatment time is about 1.5 hours.

For the patient, there are benefits to using Thermi. There’s a camera that looks at the treatment area (knees, neck, face) and gives guidance as to how much energy needs to be delivered. Also, there’s a feedback loop of the external temperature. This avoids superficial skin injury.

Overall, it is intended as a one and done treatment. No need to return for multiple treatments. Additionally, it is treating multiple areas (neck and lower cheeks) in 1 session. In fact, for treatment in the 40’s – 50’s, it can delay and/or postpone surgery.

Ear Surgery

Exceedingly protruding ears can be a source of pain and embarrassment from childhood on. Usually performed under local anesthetic, an otoplasty consists of repositioning the ears through an incision in the back. Additionally depending on your needs, your cartilage may be resculpted or removed to create a more sleek appearance. Dr. Makhlouf may also use stitches to form the ear cartilage into a more pleasing position. At our Des Plaines office we provide immediate results and most likely a rapid return to your normal activities. Ear surgery or otoplasty can typically be performed as soon as the ears have stopped growing, usually around age four. Why wait for years of negative attention and difficulty?

Prominent ears, and other ear deformities, usually present themselves at birth. An estimated 30% of these deformities correct themselves with normal aging, but a majority will require some form of treatment for correction. There are many variations, but protruding ears is the deformity that is most frequently corrected with Otoplasty.

For the first days of life, the ears can be molded. Mother’s estrogen makes the ear’s cartilage soft, allowing a brief window of time in which to reshape the ear. According to research, treatment started within the first couple of weeks after birth can be up to 90% effective, and avoid any future need for surgery. If you do notice that your newborn has an irregular shape to their ear, mention this to your doctor and see if Ear Molding is your best option.

A common time for treatment though for Prominent Ears is between the age of 5 and 6. By this age, the ear is close to 85% of its adult size, and if it was going to correct itself spontaneously, it should have by this time. Otoplasty, is performed as an outpatient procedure under general anesthesia with good long term results. The head is often wrapped for a week, and a head band worn for a few months overnight, which helps protect the results while the ear shape becomes set.

Otoplasty though can be performed at any age. A very common time for Otoplasty is in young adulthood. The ear re-shaping surgery is performed in much the same way as it is in childhood, though it can be performed under local with light sedation for the properly selected patient. If the idea of being awake causes anxiety, anesthesia may be the better option.

The most common reasons for a prominent ear are:

  • Incomplete folding – leaving the rim sticking out
  • A large central bell of the ear (concha)
  • Anterior rotation of the ear

Each of these deformities can be addressed during Otoplasty. Incomplete folding can be corrected with sutures, or by scoring the cartilage. A oversized concha can be surgically reduced, and an anteriorly rotated ear can be set back. Most of these corrections are done through a small incision hidden in the fold behind the ears, so it remains hidden, even to those with short hair.

Thermi Skin Tightening

There are a multitude of approaches to skin tightening. On the one end of the spectrum are creams, lotions, and potions, and the other end of the spectrum is a facelift procedure  (rhytidectomy). There are inherent and significant limitations to lotions and potions, and there are significant hurdles to signing up for a facelift. For a number of years, there has been interest in intermediate modalities.

Types of Skin Tightening

Laser skin tightening has been used since the mid 90’s to minimize deep wrinkles. Although effective, the multitude of laser frequencies and modalities speak to the fact that it has not yet been perfected. Ultrasound is another modality and is used for skin tightening. By penetrating the skin and heating the deep skin layers, it encourages collagen production.

There are inherent limitations to laser and ultrasound technologies in the fact that both have to go through the skin to deliver the energy needed to remodel the deep tissues. Hence pain and the possibility of permanent skin damage in terms of scarring, hyper- and hypopigmentation limit their impact on the rejuvenation.

There is now a long established technology that is being used in novel way: Radiofrequency; now used in a minimally invasive way. This refers to the use of a fine probe that penetrates the skin. Its insulated tip delivers a controlled quantity of energy to the deeper layer of the skin. The radius of impact is small, and a series of spots under the skin are needed. There is both external and internal monitoring of the temperature, a unique feature of this technology. Because it is done under local anesthesia, it is an office procedure and requires 24 to 48 hours of down time.

Cellulite is a term that originated in France in the early part of the 1900’s.

Especially in lower regions of the body, fat deposits develop between muscles and skin. Skin is attached to the muscles beneath by fibrous bands, and cellulite occurs as a result of one or more of the following reasons:

  • The fibrous bands are tight and inelastic, pulling the skin toward the muscles
  • Excess fat bulges through to skin that is not tightly tethered to muscles beneath
  • Skin is thin and loose, allowing fat pockets to push outward and make dimples

Treatment addresses one or more of these mechanisms:

  • Releasing the tight fibrous bands that cause dimpling
  • Decreasing the volume of fat through liposuction and/or weight loss

What is a successful consultation?

A successful consultation has to have specific outcomes that will be achieved. But before we get to the discussion of what these should be, both the patient and the office have to be prepared for the interaction.

The patient gets prepared by investigating the office, investigating the practitioner and checking the credentials, certificates, reputation and the experience, and preparing question they want to cover during the visit.

Dr. Makhlouf first evaluates where the stretching of the abdomen has occurred: the upper, central, or lower abdomen. The first two layers of the repair are made with specialized sutures that are placed independently of each other in two separate rows. These special sutures take into account that the stretching of the abdomen occurred in all directions. The Incision

How much does a tummy tuck cost in Chicago?

Does a tummy tuck leave scars?

Yes. The scar with an abdominoplasty will run from hipbone to hipbone, but it is easily concealed beneath underwear and most styles of bathing suit bottoms. A mini abdominoplasty will result in a smaller scar than a traditional abdominoplasty.

How long is tummy tuck recovery?

The recovery period varies from patient to patient but most can resume light activities a few days after tummy tuck surgery. The majority of patients can return to regular activities within a few weeks.

Are tummy tuck results permanent?

Tummy tuck results are long lasting unless the patient gains a significant amount of weight or becomes pregnant. Maintaining a healthy diet and exercise routine can help prolong the best results.

An Abdominoplasty (Tummy Tuck) is performed as an out-patient procedure in Chicago (Swedish Covenant Hospital) or North Suburb Des Plaines (Holy Family Hospital), Illinois.

The hallmark of a cosmetic surgery procedure well executed is the absence of an operated look. When it comes to abdominoplasty (tummy tuck), it is the umbilicus in the middle of the abdomen that attracts attention. Every effort should be made to restore it to a youthful aesthetic appearance.

The aesthetic and youthful umbilicus:

1. Is small.

2. Has a vertical orientation.

3. Has a superior hood that allows for placement of jewelry, if desired.

4. Rests in a peri-umbilical depression to provide proper contour to the abdomen.

5. Is devoid of protrusion (and hernias).

6. Allows for proper hygiene.

Most of the time, the surgeon is not able to do a proper abdominoplasty (tummy tuck) without an incision around the umbilicus, so that as the skin is pulled down, the umbilicus is allowed to remain in its anatomic position and is not pulled towards the pubic line. Most pictures showing abdominoplasty (tummy tuck) procedures show a circular scar. The problem with a circular scar is that it tends to contract and frequently remains visible. Over the years, we have relied on sound plastic surgery principles to avoid this problem. We do not do a circular scar.

Combining these aesthetic goals and plastic surgery principles, we perform the procedure in the following way:

1. The marking of the skin around the umbilicus is two-inverted V-shaped lines connected at the tip. After the incision is made, the umbilical stalk is freed from the surrounding skin and fat all the way to the abdominal wall.

2. An incision is made in the suprapubic area and the skin (without the umbilicus) is elevated and separated from the abdominal muscle all the way to the lower ribcage.

3. Tightening of the abdominal wall muscles is performed.

4. Excess skin is resected.

5. The new location of the umbilicus on the pulled skin is determined.

6. An inverted V incision is made and the umbilicus with its stalk is delivered through that incision.

Tummy Tuck Now, Beach Body in 6 weeks…

Consider the possibility of getting a tummy tuck now and having a beach body in 6 weeks. Have you been trying to get rid of that extra skin that’s sagging, after giving birth to children? Or, have you successfully lost a significant amount of weight? In either case, kudos to you.

It’s quite possible that although you work out, you may never be able to tighten what’s been undone. Childbirth has resulted in a little skin laxity, muscle laxity, as well as some extra unwanted fat. If you only have a slight amount of laxity, perhaps a mini tummy tuck is sufficient for you. The incision is smaller, however the amount of abdominal tightening is minimal, as well. With the mini tummy tuck, only the lower abdomen’s extra skin is reduced.

With a full tummy tuck procedure, the full abdomen is under consideration. The incision is made at the pubic hair line. The scar is from hipbone to hipbone, and well concealed within the bikini lines. The loose hanging skin would be removed and if necessary, liposuction would occur.

As abdominal muscles may have been loosened by pregnancy or significant weight gain, the muscles require tightening. Dr. Makhlouf has mastered the art of enhancing one’s figure in this manner. For tightening during a tummy tuck, he uses special sutures that repair the muscle weakness, in both the vertical and horizontal directions. The relevancy, here, is that when the muscles were stretched, they did so in both directions. So, it’s important to reduce the risk of this recurring.

After viewing many pictures online, one patient remarked that this is notably one of the main reasons why she came to him. He takes great care to ensure that an unnatural umbilicus is avoided. What is a mini abdominoplasty?

I have to agree that it is a confusing term. There is no standard agreement as to what is involved. This is what makes it dangerous. It allows anyone to come up with their own definition, confuse consumers, and take advantage of the confusion created.

For some a mini abdominoplasty is a small, ill-defined procedure on the abdomen to improve its appearance. There is intentional vagueness as to what the intervention entails in order to take advantage of the patient.

For some a mini abdominoplasty is standard abdominoplasty, with the procedure done on someone with a small deformity and through a smaller incision.

For some a mini abdominoplasty is a procedure that concentrates its effort in the area of the lower abdomen between the umbilicus and the pubic hairline, tightening the muscles of the lower abdomen and removing excess skin.

For some a mini abdominoplasty entails the excision of skin from the lower abdomen, and on occasion liposuction of the upper abdomen is included, but there is no muscle correction.

These various possibilities create confusion as to the surgical time the procedure takes to perform, the recovery time involved, what outcome is to be expected and what a fair cost should be. The lack of clarity prevents interested people from having a clear value proposition.

In order to properly define a “mini abdominoplasty” let’s first identify the elements in a standard abdominoplasty and see which ones are included in a mini-abdominoplasty.

In a standard abdominoplasty:

1. Excess skin is removed through an incision at the pubic hairline.

2. Muscle relaxation and the occasional hernia are corrected by tightening the abdominal wall muscle and providing a strong platform on which to re-drape the skin.

3. As the skin is pulled down the belly button (umbilicus) has to be addressed. It cannot be eliminated nor can it be pulled down towards the hairline. So a specialized incision needs to be made around the umbilicus leaving it attached to the abdominal wall like a flower on a stalk. The abdominal skin is separated from the umbilicus and the abdominal muscles underneath, and an attempt is made to remove then skin segment between the belly button and the pubic hairline. The belly button is then brought out through an incision in the pulled down skin.

Sometimes included in an abdominoplasty is the suctioning of the upper abdomen and sides.

For everybody the relocation of the umbilicus on the abdominal wall skin as part of a standard abdominoplasty. The mini abdominoplasty does not involve an incision around the umbilicus in order to allow its relocation.

What are the choices in a mini abdominoplasty?

Type I involves a skin excision over the pubic hairline with or without liposuction of some part of the abdomen.

Type II involves type I and tightening of the abdominal muscles below the umbilicus without its relocation.

Type III also called “umbilical float” procedure involves keeping the umbilicus attached to the skin around it separating the whole abdominal skin and bellybutton from the muscles underneath tightening the abdominal muscles. As the skin is pulled down the umbilicus is relocated one inch lower than its original position and extra skin is removed at the pubic hairline incision.

Buttock Augmentation

Q: Is there a difference between buttock augmentation and buttock lift?

A: There may be some redundancy of the skin, but the buttock augmentation patient is seeking mainly fullness in the region, whereas the buttock lift patient wants to get rid of the redundancy that has occurred due to aging or weight loss and have their butt lifted. A small amount of redundancy can be corrected by increasing fullness with an implant, but a more significant redundancy requires a longer incision.

Q: How can buttock augmentation be performed?

A: There are three ways to perform buttock augmentation.

  1. With a butt implant.
  2. Imbricating, (meaning folding) the extra skin and fat in the buttock area. This is obviously a limited procedure reserved for massive weight loss patients.
  3. Fat transfer involves liposuctioning from one region of the body and transferring to the buttocks.
  4. There can be a combination of any of these to lift.

Q: What are the long-term effects of buttocks augmentation with an implant?

A: The implants are made of a very soft solid silicone material and cannot leak or break down. There would be no reason to have the implant changed after a certain period of time.

Q: What are the long-term effects of buttock augmentation with fat transfer?

A: Buttock augmentation with fat should be stable over many years and we feel that after the one year mark has passed, the amount of fat that is remaining should be stable; however, it is subject to weight loss and gain. Please review the section of pros and cons of every procedure.

Q: What are the pros and cons of a Butt Augmentation with Implants?

A: The pros of an implant are:
- Similar controllable amount on both sides.
- It can be used in skinny people.
- Short operating time (two hours).
- Reduces cost.
- Will not fluctuate with weight loss or gain.
- Implants are “permanent” and do not need to be replaced, as they are soft and solid.
The cons of an implant are:
- There may be a little more firmness than a normal buttock.
- Limitations as to the size that can be put safely.
- An approximate 7 cm vertical incision between the buttocks.

Q: What are the pros and cons of fat transfer?

A: The pros:
- You get liposuction of unwanted fat if present and correction of more than one area.
- The patient’s own tissues (fat) are used.
- It feels like fat.
- Tiny invisible incisions.
The cons:
- A longer operating time.
- Cannot predict how much fat will survive.
- May develop some hard lump.
- Not enough fat may be available for the procedure.
- Increased cost primarily related to surgical time and the need of related procedure.
Generally with fat transfer more than one session needs to be planned.

Q: What are the pros and cons of fat imbrication?

A: The pros:
- Two problems can be corrected at the same time: the redundancy and the lack of projection.
- Survival of the fat is less of an issue than with fat transfer.
- Shorter operating time than fat grafting.
The cons:
- Longer scar.
- Limited augmentation. We just double the skin thickness at best.
- Sagging might recur.
- No long-term studies.

Chicago Buttock Augmentation

Thirty years ago, physicians were placing breast implants in the buttock area in order to increase fullness and projection. Unfortunately, this procedure was fraught with complications; some directly related to the implant itself (rupture of silicone, limitations in shape and size of the implant design) and some due to the technique and location of placement. The implants were placed subcutaneously and frequently resulted in an unacceptable shape because of the scar tissue that would form.

As plastic surgery evolved, silicone became available as a soft solid with different shapes and sizes, and liposuction and fat grafting have become popular.

Our aesthetic goals have evolved over time and include the whole area of the lower back, buttock and upper thighs addressing proper projection and fullness. The maximum projection should be at the level of the pubic hair when viewed from the side. The maximum fullness from the back is not in the middle of the buttock when viewed straight back, but in the middle of the buttock when viewed from a 45 degrees angle to the back. The scar has to be inconspicuous. We need to incorporate the buttock with the flank and hip areas in our evaluation, and we have to take into account the trochanteric and thigh areas and decide if they will require special attention.

Until a few years ago we were able to offer only liposuction for this area of the body. Today along with liposuction we can add either an implant or the fat. When fat is used it is harvested through liposuction and then introduced in the buttock area through small cannulas.

In order for the fat to survive, it has to be injected in very minute quantities so that the surrounding tissues can provide nourishment for these cells. If a large volume is injected in any one location, the central part of this imaginary ball of fat and fluid would not be in direct contact with the tissues and not have access to oxygen and nourishment; the fat cells would then die. Because of this limitation, multiple tracts are created in the tissues of the buttock in order for the surgeon to be able to deliver the amount needed. Extreme care with regard to sterility of the procedure must be maintained. Antibiotics are a central component of this procedure. The first limitation for fat injection (grafting) is the amount of fat that any individual patient has available for use. The second relates to the fact that less than 50% of what is aspirated can be used in the augmentation, and the third is that 30-50% of that added volume might be lost over time.

If buttock augmentation is going to be achieved with a silicone implant, the procedure is done through a very specialized incision in the fold between the buttocks. It is designed to prevent separation of the tissue and to prevent accumulation of fluid between the two buttock cheeks that could lead to problems with healing. After this incision is made, a pocket is dissected to the exact size and shape of the implant.

As mentioned earlier, the implants we use today are specific for the buttocks and proper anatomic placement is crucial. Earlier in the learning curve the implants were placed subcutaneously often resulting in the previously mentioned problem. Subsequently, the implants were placed underneath the muscle, but that position suffered two setbacks. The first was that the implants were riding higher than aesthetically desirable and when the surgeon tried to bring the pocket a little lower, pressure on the sciatic nerve was a potential complication.

Today we place the implant in one of two ways. One is within the gluteus muscle itself. The other is underneath the fascia of the muscle. This is the location that I use because it provides me with the flexibility that I need and minimizes the discomfort. The patient is marked before surgery in the standing position with the help of a sizer and dissection adheres strictly to the markings. We inject the area with a long acting pain medication at the onset of the procedure. This seems to help the patient significantly with postoperative pain. A garment is placed at the conclusion of the procedure. Care is simple and within two weeks scar tissue has formed around the implant keeping it in position.

Now that we are beyond the learning curve, I am able to offer a reliable procedure with very few risks, little discomfort and a short downtime. It has been rewarding both for me and the patients who walk away with an increased confidence in their transformed appearance.

The Brazilian Butt Lift: What’s in a Name?

The Brazilian butt lift is a very popular procedure; in fact it is the fastest growing form of cosmetic surgery today. If you’re considering surgery to round out a backside that appears flat, square or simply too skimpy, it’s critically important to choose a plastic surgeon who is very experienced. Here’s why.

The Brazilian butt lift is not truly a lift. Muscles, fat and skin are not elevated in the same way that facelifts and breast lifts raise sagging tissues. A well-done butt lift appears to raise the buttocks because increased volume enables the skin to re-drape more tightly. There are only two ways to increase the size of the buttocks, and both present challenges for an inexperienced plastic surgeon.

Fat Transfer

Many physicians favor fat injections to add volume to the buttocks. This approach can be successful for a patient who desires a modest increase and has enough fat available from other areas of the body. After harvesting and processing, fat is transplanted in small quantities so surrounding tissues can nourish it. Some fat is lost during harvesting and some does not survive after transplanting—it is wise to plan for at least a 40% loss. A typical 400 cc result for each buttock requires more fat than many patients have available.

Silicone Implants

Silicone implants are a good alternative for many women. The material is a soft semi-solid that will not leak. The implants are safe, do not need to be replaced and feel natural.

The challenge with implants is that some plastic surgeons only use up models up to 300 ccs in size. Larger implants mean wider pockets must be created through relatively small incisions with restricted visual access—a procedure that’s problematic for many surgeons. Dr. Makhlouf developed and patented special instruments for this purpose, tools that expose the proper amount of tissue, flood the area with light and allow for efficient suctioning. These instruments enable Dr. Makhlouf to place implants up to 500 ccs in size. His patients are pleased with their results, happy with both projection and symmetry.

Catchy procedure names abound in the world of cosmetic surgery today, and the Brazilian butt lift is no exception. Not only is the lift aspect of the procedure not strictly accurate, there’s really nothing Brazilian about it. What does “Buttock Augmentation” really mean?

Ok, so you’re confused by all the different terminology in and around Buttock Augmentation. Who isn’t?!!! We’ll simplify the murky waters, here, and define the different procedures. On a positive note, there are only a handful of procedures:

Incision: There will be injections, no incision.

Procedure: Fat is harvested from other areas of the body and injected into the buttocks.

Buttock Augmentation:

Goal: Add volume to the butt that is otherwise flat or not full enough.

Incision: At the intergluteal cleft: the groove between the butt cheeks.

Procedure: A soft and solid implant is inserted into the buttock area.

Result: A full and soft butt that will match the rest of the body.

Labiaplasty

Labiaplasty is a popular procedure that’s sometimes incorrectly referred to as labioplasty. It started as a natural progression over the last decade. Women had been increasingly paying attention to the area of their body covered by a bikini. It started with trimming the hair, then shaving it, and now to permanently removing part or all of it. The advent of the laser and the various reality shows have made it mainstream. There are even articles written about it. In fact, the American Society of Plastic Surgeons (ASPS) notes an uptick in labiaplasty procedures: a 39% spike in 2016, with more than 12,000 procedures.

Background & Concerns

The logical progression was to pay attention to the plumpness of the mons pubis, the rounded mass of fatty tissue that’s over the pubic bones (as per Wikipedia). In recent days, women have started to pay attention to the fullness, or the lack thereof, of their labia majora (outer vaginal lip). Women are also concerned about the size and shape of their labia minora (inner vagina lip). Here’s a great anatomical pictorial identifying the different parts of the female reproductive organ.

The Labiaplasty is a procedure that is directed to the labia minora to address concerns about size, fullness, redundancy and hooding. Some women feel uncomfortable during sexual intercourse, some feel self-conscious during exercise, and in a swimsuit or weight lifting attire. Others also have pain with certain activities like riding a bicycle or horseback riding.

Procedure

Based on the physical examination, various techniques can be used to address the patient’s concerns. Along with the appropriate choice of technique, careful and delicate suturing provides for very satisfying results. The goals are to:

  1. Provide inconspicuous correction.
  2. Minimizes the chance of scar hypersensitivity and painful intercourse.
  3. Heal rapidly.

The procedure can be done in the surgery center or the hospital if there is a request for sedation. But the majority of our patients are comfortable with the procedure done in the office operating room under local anesthesia. It only takes 45 minutes to perform. A very small amount of pain is involved. We do our utmost to provide our patients with a comfortable experience away from any embarrassment.

Post-Op

The post-op care is very minimal and easy to perform. Activity restrictions are:

  • No sexual intercourse for 3 weeks
  • No bicycle riding or horse-back riding for 3 weeks
  • Further, the follow-up appointment schedule is for 5-7 days, 3 weeks, and 2 months.

Liposuction

It’s no surprise that abdominal liposuction is one of the most popular cosmetic surgery procedures in the United States. The tummy and the love handles seem to plague even the most fit and beautiful people and a lot of that is because abdominal fat can be some of the most stubborn and difficult to burn.

Well, the good news is that you don’t necessarily have to turn to a full on tummy tuck to get some good results. In fact, quite often, abdominal liposuction will give you results that will please you, with less risk, less recovery time, and a lower cost.

If the procedure is successful, you will have spent less time and less money on a less risky procedure and you may achieve satisfactory results. If the abdominal liposuction is insufficient, you’ll have completed the first step of your tummy tuck anyway, which is fat removal, and you can move forward with your Abdominoplasty.

Liposuction or Tummy Tuck?

Many patients are unable to decide what the best procedure for them in order to reduce their abdominal girth. The easiest way is to look at what causes the bulge. The fat present behind the muscle and with the intestinal organs cannot be reached with liposuction. A low carbohydrate diet is a way to reduce that type of fat. Liposuction of the skin fat is possible provided there is enough fat to make a difference.

The only time a tummy tuck is required for a man is if he has lost a significant amount of weight. There will be excess skin removed and the abdominal wall muscles would have been stretched by the excess weight in the skin the man had been carrying around.

Everyone remembers the moment, early in a relationship with a new boyfriend or girlfriend, when you compare scars and tell the stories of how you got them. No one wants to say, “I just got that one after my liposuction surgery.” That’s a reason why one of the fears that patients have about undergoing liposuction is that there might be permanent scarring. Recovery

The first question most patients ask about their body contouring surgery concerns the recovery period. It is important to discuss your recovery expectations with your doctor, as depending on the duration and extensiveness of your procedure; recovery time could be lengthy and involve the assistance of friends or family. Dressings and/or bandages will be applied to all incisions, possibly along with small tubes placed under the skin to drain excess fluids or blood. 3 weeks after surgery: You can go tanning and swimming!

You will be able to go swimming because the incision will be healed three weeks after surgery.

If you chose to tan 3 weeks after, make sure to protect the incision with sunscreen that is SPF 15 of higher so the scar won’t become dark.

 

8 weeks after surgery: Swelling will go down and you will have an accurate idea about your new breast size!

That means 8 weeks after surgery it is time to go shopping and buy new bras, swimsuits, and other clothes! It is essential that any decisions that take into account your breast size, such as a wedding dress, should not be decided until 8 weeks after surgery.

 

Factors to consider before having a breast augmentation:

1. Pregnancy

A breast augmentation will NOT affect your pregnancy. You will still be able to breast feed. Pregnancy, however, does affect one’s breasts. After being pregnant some women’s breasts get smaller, others bigger, and others droopier. That means if you get a breast augmentation and soon after get pregnant your breasts may change and you could need a revision surgery to get them back to how they were before you were pregnant. So if you plan on getting pregnant take that into consideration while thinking about your breast augmentation surgery. You can even wait until having your child or children and add a tummy tuck and get what we call a mommy makeover!

2. Weight loss

Weight loss can cause one’s breasts to decrease in size or can result in droopy breast due to there being more skin than there is fatty tissue in the breast. That means if you are planning on losing weight you should ideally do so before your breast augmentation. The reason for this is so that the excess skin can be factored into the surgery and taken care of so you do not finally have the breasts you want only to lose weight and then have your breast droop. During consultation for breast augmentation there are essentially three decisions that the doctor and the patient have to make:

1. The type of implant: saline or silicone

2. The size of the implant

3. And whether to place the implant above the muscle or below the muscle

The decision process can be made starting with any of those issues. But it is important to understand that any decision we make will affect one or both remaining issues. A decision in one affects the choices in the other; therefore, they cannot be made independently of one another.

For example, starting with decision one: saline or a silicone implant.

We know from a previous blog that a silicone implant before it is inserted in a patient is softer than a saline implant. The saline implant has more projection per volume than a silicone implant (which is why the companies are coming up with firmer types of silicone implants to increase the projection similar to a saline implant).

Let us assume you decided to use a saline implant, the next question will be: Is there adequate cover for the implant? Adequate coverage is defined by having enough breast tissue to avoid the result from being an unnatural round ball on the chest, and hiding any rippling of the implant. If coverage is deemed insufficient particularly in this superior pole of the breast then one will have to resort to a sub muscular placement. Here, a decision in issue number one has affected issue number three.

On the other hand, if a silicone implant is chosen and you want projection, then a larger implant needs to be selected.

Here we see that the type of implant is impacting on the size of the implant.

Let us take another example: Looking at the implant size, the goal of using a large implant is to enhance projection. However when a large implant is chosen, your doctor needs to assess whether there is enough tissue to provide adequate coverage for the implant. If that is not possible then one might recommend a saline implant to obtain a better projection with a smaller volume (issue number1). So in this example the size of the implant has an impact on the type of implant

Now if we insist on a larger On the other hand, if you insist on a large implant and the type is saline implant, it will have to be placed in a sub-muscular position to improve coverage. (issue number 3).

Again here making choices in one and two has affected the choice we have in issue three.

Because of the complexities of the decisions, it is important to have an experienced surgeon and one who takes the time in the consultation to make the right choice for you. It should not be relegated to a staff member other than your surgeon. In Dr Makhlouf’s office the decision is shared between the doctor and the patient and the thought process explained.

How do I prepare for a breast augmentation procedure

Dr. Makhlouf will provide thorough preoperative instructions, answer any questions you may have, take a detailed medical history and perform a physical exam to determine your fitness for surgery.

 

To help detect and track any changes in your breast tissue, your plastic surgeon may recommend:

A baseline mammogram before surgery
Another mammogram a few month after surgery
In advance of your procedure, Dr. Makhlouf will ask you to:

Stop smoking at least 6 weeks before undergoing surgery to better promote healing.
Avoid taking aspirin and other certain anti-inflammatory drugs that can increase bleeding.
Hydrate, regardless of the type of surgery, hydration is very important before and after surgery for safe recovery.
For breast augmentation using autologous fat transfer, you may be instructed to wear a special bra to expand the skin and tissue around the breasts to prepare them for fat injection.

Breast augmentation is usually performed on an outpatient basis. Be sure to arrange for someone to drive you home after surgery and to stay with you at least the first night following surgery.

For many years, fat transfer from one part of the body to the other was thought to only be a temporary correction. However, over the last few years a large body of evidence, both scientific and photographic have shown that if the fat is transferred properly, it can result in a permanent solution.

Early on there was a lot of fear that fat placed in the breasts would melt away, possibly create cysts, and more importantly create calcifications that would hide a present cancer by decreasing the efficacy of mammographic detection. There have been multiple reports in the literature which indicated that a radiologist is in fact able to distinguish between malignant calcification and benign calcification that could have resulted from fat placement in the breasts.

Studies have also shown some amazing results which support that fat grafting has improved the quality of the skin under which it was grafted. The safety of the procedure along with its incredible results have brought fat grafting to the breasts into the cutting edge of cosmetic surgery.

On a trial basis Dr. Makhlouf has injected breasts with fat obtained through liposuction and has monitored the results. He was performing this trial of fat grafting to the breasts while doing another main procedure on the patient. He has been able to document survival of the fat and the patients have been quite happy.

It is worth mentioning that it is impossible to place large amounts of fat in the breasts in volumes similar to the sizes of breast implants. Many patients who desire breast augmentation want breast implants of 300 to 400cc in volume. Fat grafting to the breasts cannot be expected to add to more than 200 cc in volume, therefore, a repeat procedure would be needed if one wants the augmentation done with fat alone.

Patients interested in breast enhancement are frequently confused about the choices and options available to them regarding the type and size of implant, location of implant placement ( under or over the muscle) and the type of incision. There are many reasons for the confusion, one of which is because questions cannot be answered independently of one another and therefore various decisions need to be made simultaneously. The other reason for confusion is that much information is biased and opinionated and does not allow for a rational decision.

I am going to start with the most difficult issues regarding the type of implant to use, saline versus silicone and its placement, above or below the muscle.

Let’s provide some factual information:

1. Saline implants require a smaller incision.

2. Saline implants provide flexibility in scar location.

3. For a given final volume, the envelope in a saline implant is smaller than that of a silicone implant resulting in a firmer implant as it lies outside the body.

4. Saline implants placed under the muscle tend to feel softer than above the muscle.

5. The firmness of the breasts is directly related to the scar tissue formed around the implant, inversely proportionate to the amount of breast and muscle tissue in front of the implant and the inherent firmness of the implant.

6. When an implant is placed underneath the muscle only the upper inner part of the implant is covered with muscle. The outer and lower part of the implant is covered by whatever breast tissue, fat, or skin is available.

7. Silicone implants are much more amenable to placement above the muscle.

8. Silicone implants are more expensive than saline implants.

9. Silicone implants are felt to be longer lasting than saline implants, but the present type of implants have been on the market only since 2006 and have been implanted in trial since 1992.

10. Implants placed subpectorally have three drawbacks:

  • They are more painful in the immediate postoperative period.
  • They can occasionally result in distortion of the breasts with muscle contracture.
  • If the patient has some breast tissue and the breasts themselves are soft or become softer over time, the breast will tend to drop down while the implant stays in its initial higher position under the muscle. However, an implant placed underneath the breast tissue will tend to migrate down with the breast tissue, maintaining the aesthetic result longer.

11. Cleavage issues: the implant must be centered underneath the nipple; therefore, the cleavage is directly related to the distance between the sternum in the midline and the nipple itself, and the size of the implant to be placed in that space. As the implant gets larger its radius becomes larger and if the center of the circle is at the nipple, then the larger radius will allow for more fullness medially and, therefore, more cleavage.

12. In order to have a satisfactory result with implant placement above the muscle, 2 cm of breast tissue in the upper pole are needed. With silicone implants, we are not as strictly restricted as with saline implant by this law.

13. Mammography: The sensitivity of a mammography decreases with any type of implant in any position. Generally, radiologists prefer implants placed subpectorally.

MRIs are very valuable if implants are present and are required for patients who have silicone implants.

14. Incision site: there are four types of incisions available.

The inframammary incision is the most common incision and is felt to be the safest. The next common incision is a periareolar incision. The way I perform the enhancement, is by making the incision in the lower part of the areola elevating the skin off the lower part of the breast and then going down underneath the breasts with a similar dissection as an inframammary incision. The axillary incision is performed less commonly. Its drawback is the inability to always properly dissect the lateral pocket to provide a natural result. If bleeding were to occur, it will occur farthest away from the entry site and may require a separate inframammary incision for control. Sometimes curvature of the chest prevents the surgeon from visualizing well the medial and inferior aspects of the pocket. Therefore, I personally reserve it to a situation where small implants are going to be placed. The last incision is from the umbilicus. This does not allow for silicone implant placement and voids the warranty that the manufacturer provides. I do not offer it to my patients.

Implant size: the way we manage size is a combination of the patient’s desires, the result of sizers tried in the office at least two times and the measurements we obtained during the examination. All that combined with my surgical experience dating back to 1987 is used to help the patient chose an implant they are happy with.

In “20 Things You Never Knew About Botox”, the author defines the origins of Botilinum Toxin, commonly known as Botox. Apparently, it was initially used to treat a muscle-related eye condition. The patients were pleas

ed that their wrinkles were less apparent. They then realized that it could potentially have multiple applications – face and neck spasms, excessive sweating (hyperhidrosis), and for treating migraines.

The Numbers Speak…

The American Society for Aesthetic Plastic Surgery (ASAPS) has recently reported that the number of cosmetic procedures are on the rise. It is the statistics (2017) report from the American Society of Plastic Surgeons (ASPS) that shows:

  • Botilinum Toxin Type A procedures increased a mere 2% from 2016
  • Their total number of procedures in 2017 was 7.2 Million.

While any time of year is a great time to start your journey towards a new you, many plastic surgery patients are surprised to find that, contrary to what many may assume, winter – not spring or summer – is a very popular time to undergo a variety of procedures. There are several reasons, listed below, for why fall and winter are the best times to undergo cosmetic surgeries, and we invite prospective patients to call our offices to begin the process of self-transformation in winter of 2016.

Timing your procedures to provide optimal results during summer.

Healing takes time, and while you may see incredible results not long after your procedure has been completed, many surgeries require weeks or months of recovery time before all incisions are healed and swelling has diminished entirely. While mobility is shortly restored to most patients, risk of tearing sutures or causing other damage may require them to avoid certain activities.

Patients who undergo their surgeries in the fall or winter stand an excellent chance of fully enjoying their results by summer, a full 4-6 months after their procedure and recovery is complete.

Easier concealment of swelling and bruising.

Bruising, swelling, and visible incisions are temporary and an inherent part of cosmetic procedures, but that doesn’t mean patients embrace showing them off! Hot weather can make it more difficult to fully hide visible evidence of your recent procedure, as it is uncomfortable to wear long-sleeve shirts, turtlenecks, long pants, or other clothing that is helpful in concealing these marks. Winter, however, calls for a wardrobe that is immensely effective in helping to keep patients’ medical procedures private.

A better time recovering.

Recovering can prevent individuals from doing much physical activity or, in some cases, from leaving their home for a period of several days, which can be distressing to some. Few similar activities are available during a Chicago winter, helping to prevent our patients from feeling as if they are “missing out” because of their surgery.

Favorable weather conditions.

Blepharoplasty: A corrective eyelid surgery on either the upper or lower eyelids. The procedure removes excess fat, skin, or muscle.

Cautery: Is an electrical device that generates energy denatures protein and causes them to clump together. If applied to a blood vessel, this process seals it and prevents further bleeding.

Crow’s Feet: The lines that fan from corner of the eyes at the junction of the upper and lower eyelid are called crow’s feet. They are accentuated by smiling and are also called smile lines.

Glabellar Area: The 11 lines that are formed between the eyebrows.

Hemostasis: A process which causes bleeding to stop. It is achieved in the operating room either by tying the blood vessel off or coagulating it with cautery.

Nasojugal Fold: Otherwise known as the tear trough, this is the fold that is obliquely extended from the inner corner of the eye and delineates the union between the skin of the eyelid and the skin of the cheek.

Nasolabial Fold: This is the crease that begins at the bottom of the nose and extends to the corner of the lips.

Orbit: The orbit is the bony cavity of the skull in which the eye, the muscles and the fat are located.

Platysma: The Platysma is a muscle present between the deep structures of the neck like the nerves, carotid artery, trachea, etc. and the skin.

Ptosis: Ptosis is the droopiness of the eyelid. It usually refers to the upper eyelid. It can be congenital, or due to muscle weakness or separation of the muscle from its attachment in the eyelid cartilage.

Radiesse: An injectable, made from calcium hydroxyapatite, a molecule found in bones and corals. Its particles are small enough to be injected and add volume to the area.

I am going to explore how I maximize the numerator and minimize the denominator.

I am a board certified Plastic Surgeon that has a teaching appointment at the university of Illinois. I take the residents through difficult surgeries, and attend conferences locally and nationally to maintain proficiency and compare my results to others.

I have been in practice since 1987, I have a lot of experience seen procedures in their infancy and development over time, I have seen the learning curve, the mistakes, and the procedures that survived and those that did not. So this gives me a unique appreciation of new technologies, their benefits and shortfalls, their place in the armamentarium of a good plastic surgeon and how to use them best.

I work hard for every patient and I never approach a surgical operation as a routine procedure, I constantly strive to prepare for every procedure ahead of time. Moreover, I perform the surgery in my head at least once before I perform it on the patient. My work ethics combined with a multitude of procedures performed over the years gives me an edge on the other practitioners.

I wish I could say that all my procedures come out perfect every time, but I am only human. However I am constantly comparing my results with those published in the literature, and many times I come ahead or equal. If on occasion I am not getting the results I like to see, I will research why and how I can make my outcomes better. If I am not successful at getting quality results I simply will not offer that particular procedure. This way I concentrate on a panel of successful procedures and make them available to my patients.

I understand that the patient is looking for more than just a transaction; they are looking for a relationship- a comfortable and trusting interaction with the practitioner. Knowing this fact I strive to provide it. I will not manipulate patients and remain genuine so they will have only the procedure they need, I will not encourage them to have more procedures than they want unless it is medically or aesthetically beneficial. I will explain my reasoning and recommendation. Whenever possible I will give them the least costly alternative, rather than the more expensive one. My price structure is set up to reinforce this commitment. I do not look at my patients as customers that I want to make the most money from, but rather as patients, human beings asking the help of another human being.

I am a perfectionist meaning I am never easily satisfied. I am always striving to improve my outcomes, and if I feel I could give a patient a better result than what was achieved, or if in my mind the outcome should have been nicer, I will try to fix it with the least possible cost. My results and “batting” average speak in my favor.

I always want to be up to date with the scientific knowledge. I attend meetings every year, I stay up to familiarize myself with the medical literature, and try to publish if whenever I find something that could be of help to other practitioners. I do not publish to show off but only if I have something relevant to say. The process of publishing entails a thorough knowledge of other publications which forces me to become an expert in the subject.

Physicians are required to maintain up to date knowledge by obtaining yearly education credits through exams, reading of the literature or other medical education activity. I have always more Continuing Medical Education credits than required, and because I pay for that from my own pocket, those credits have always been relevant to my practice and to the knowledge I feel I need to have to remain an up to date physician.

I have always been keenly aware that if one stops this process of self-education one becomes irrelevant quickly. Plastic surgery has been changing and developing very fast during my 25 years of practice, and this is how it has been able to grow in spite of the economic challenges and encroachment by other specialties and untrained practitioners. (There are in the Chicago area radiologists performing liposuction, otolaryngologist performing breast augmentation and abdominoplasty, ER physicians performing facelifts etc…)

As to what the cost is:

My fees are average or just below the mean compared to other qualified physicians in my field. I try to maintain a small overhead, and the office provides financing ability. The facility fee I have negotiated on behalf of my patients with the hospital is very reasonable, and less expensive than many surgi-centers. It has an invaluable safety and resources needed in term of supply and equipment. The setup is very private and comfortable, all, of this offers an added value too.

Another unaccounted for cost is the emotional input. It is inherent to the specialty and people come with varied emotional baggage. I try to identify and understand it, and over the years of experience have developed ways to address their needs. What I have found of utmost use to help mitigate this emotional cost is to provide the detailed education as to the procedure, the possible complications and what we would do the prevent them from occurring. I also make sure we have a good rapport with the patient before the surgical procedure. I insist on a preoperative visit during which we address all the questions that might have arisen after the first consultation, I go over what is the procedure that is going to be performed, how it is going to be done, if implants are involved we try the sizes again and prepare the garments that are going to be needed post operatively. I will also review the medical history and safety issues review the lab work, give last minute instructions about how to prepare for the procedure, give the prescriptions and explain the reasons for them and how to take them. I have found this visit invaluable and take away a significant part of the emotional load that comes with the surgery.

Time to heal is an inherent biology issue that we can rarely alter, however, I see the patient very frequently after the surgery to monitor their progress make sure they are healing as they should and “hold their hand” until they start to see the expected result, I make myself available to them and call them as needed to encourage them and monitor their progress.

Obtain a Medical Evaluation: Your general health can affect the results—and safety—of the plastic surgery procedure you choose. When considering plastic surgery, consult with your surgeon for an evaluation and a full medical history to determine what is most appropriate and advisable for you.

Be Informed: Talk about the procedure with trusted friends and family as well as your physician. Prior to Surgery

Before going in for a surgery, there are few more steps to take. You’ll need to get clearance from your physician. Your clearance is usually only good for 30 days, so it’s imperative to get it done in a timely manner.

A week before surgery, you’ll come in for a pre-operative visit. It’s at this time you’ll pay for the surgeon’s fees. That amount all depends on the procedure.

Day of Surgery

It’s the day of surgery and the day of your final payment. This last and final payment will be made at the surgical center. It covers the facility fees, anesthesiologist, etc.

Is There Such a Thing as Depression After Surgery?

You’ve done your research about a plastic surgery procedure and you are ready. However, you’ve heard that there is such a thing as depression after surgery. Could it be so? Are you exempt? How can you avoid it?

When patients first come to see Dr. Makhlouf for their pre-op visit, he advises them not to do anything taxing for a week after the surgery. The onset of fatigue may occur sooner than you expected, in your daily activities. You may close your eyes sooner than you expected to. Also, watching TV or your electronic devices may be a bit of a strain. If you’ve had facial surgery, you may also experience some facial pain during the first couple of days.

Dr. Makhlouf advises his patients to expect some bruising, swelling, and being tired. If none of these after affects evidence themselves, you should consider yourself one of the fortunate few. Be prepared for the former vs. the latter.

Post Operative Care

Before your surgery, you should consider who will take care of you post operatively? Dr. Makhlouf advises that you should have a supportive and caring individual. You’ll definitely require support and someone that’s considerate of your emotions and care. Your emotional well being will vascillate, as well. The caretaker may be affected by that.

Surgery affects each person differently. The most common reaction is being depressed on the 3rd or 4th day. Some patients think they may be the exception to the rule. However three weeks later, they may have a crying bout on their way home. Sometime near the end of the second week, the patient starts to feel good. There’s also a day in there when they realize that they actually look magnificent.

Patients experience feedback from others, both positive and negative. Initially, you may feel irritated because people that are now paying attention to you didn’t pay attention to you before. It’s quite likely that this is the initial reason why got the surgery to begin with. You want to look and feel good about yourself, and you want others to notice. As per the Dr., patients eventually start to enjoy the extra attention.

Anyone who has cosmetic surgery has shown that they are a person of courage. They simply expect more from themselves, now. They’ve given up the age old excuse of, “I’m too afraid to do that.”

Plastic surgery can improve your self-esteem and enjoyment of life, according to a new study by scientists at the Ruhr-Universität Bochum (RUB) in Germany. In this long-term study, the largest of its kind ever undertaken, scientists researched the psychological effects of plastic surgery on 550 patients. The study subjects showed a greater enjoyment of life and improved self-esteem after their cosmetic surgery procedures to enhance their physical appearance. The scientists reported their results in the journal Clinical Psychological Science.

The researchers first studied whether plastic surgery patients are systematically different from other people, their goals before plastic surgery, and whether they reach those goals. There were no significant differences among the study subjects in their psychological and health factors.

RUB researchers then studied patients’ goals. About 88 percent of the study subjects offered realistic goals, citing the desire to “feel better,” “develop more self-confidence” and “eliminate blemishes.”

The researchers tested the study patients before surgery, as well as three, six and 12 months after. On average, the study patients reported they had fulfilled their goals and were satisfied over the long-term. Compared to a control group of study subjects who chose not to pursue plastic surgery, the plastic surgery patients reported feeling healthier, less anxious, and enjoyed greater self-esteem. No adverse effects were observed in any of the study subjects.

There are two reasons to ask the cost of a purchase:

The first reason is to see whether we can afford the purchase, and the second reason is to compare the purchase price at different locations.

In order to decide if one can afford the procedure, an evaluation of one’s financial situation is needed:

-Do I have the money now?

-Am I going to have unexpected expenses within the coming three to six months, or am I going to have expected increase in personal cash flow secondary to tax refund, increase in salary job, gifts, maturation of investment, or completed loan payment, for instance for the car or house?

-Am I going to receive alimony payments or settlement of probate, inheritance, etc.?

-If I do not want to deplete my cash reserve, should I consider short-term financing? This, I believe, is one of the greatest features of financing with zero or very low interest rate because it helps manage the cash flow.

In order to compare the purchase at different locations, we have to use a different concept than the pure cost of the purchase. We have to look at the value of the purchase. Value is defined as the ratio of what we are getting divided by the actual cost itself. Looking only at the cost of a purchase at different locations does not allow for proper comparison.

2. An incision is made in the suprapubic area and the skin (without the umbilicus) is elevated and separated from the abdominal muscle all the way to the lower ribcage.

3. Tightening of the abdominal wall muscles is performed.

4. Excess skin is resected.

5. The new location of the umbilicus on the pulled skin is determined.

6. An inverted V incision is made and the umbilicus with its stalk is delivered through that incision.

Tummy Tuck Now, Beach Body in 6 weeks…

I have to agree that it is a confusing term. There is no standard agreement as to what is involved. This is what makes it dangerous. It allows anyone to come up with their own definition, confuse consumers, and take advantage of the confusion created.

For some a mini abdominoplasty is a small, ill-defined procedure on the abdomen to improve its appearance. There is intentional vagueness as to what the intervention entails in order to take advantage of the patient.

For some a mini abdominoplasty is standard abdominoplasty, with the procedure done on someone with a small deformity and through a smaller incision.

For some a mini abdominoplasty is a procedure that concentrates its effort in the area of the lower abdomen between the umbilicus and the pubic hairline, tightening the muscles of the lower abdomen and removing excess skin.

For some a mini abdominoplasty entails the excision of skin from the lower abdomen, and on occasion liposuction of the upper abdomen is included, but there is no muscle correction.

These various possibilities create confusion as to the surgical time the procedure takes to perform, the recovery time involved, what outcome is to be expected and what a fair cost should be. The lack of clarity prevents interested people from having a clear value proposition.

In order to properly define a “mini abdominoplasty” let’s first identify the elements in a standard abdominoplasty and see which ones are included in a mini-abdominoplasty.

In a standard abdominoplasty:

1. Excess skin is removed through an incision at the pubic hairline.

2. Muscle relaxation and the occasional hernia are corrected by tightening the abdominal wall muscle and providing a strong platform on which to re-drape the skin.

3. As the skin is pulled down the belly button (umbilicus) has to be addressed. It cannot be eliminated nor can it be pulled down towards the hairline. So a specialized incision needs to be made around the umbilicus leaving it attached to the abdominal wall like a flower on a stalk. The abdominal skin is separated from the umbilicus and the abdominal muscles underneath, and an attempt is made to remove then skin segment between the belly button and the pubic hairline. The belly button is then brought out through an incision in the pulled down skin.

Sometimes included in an abdominoplasty is the suctioning of the upper abdomen and sides.

For everybody the relocation of the umbilicus on the abdominal wall skin as part of a standard abdominoplasty. The mini abdominoplasty does not involve an incision around the umbilicus in order to allow its relocation.

What are the choices in a mini abdominoplasty?

Type I involves a skin excision over the pubic hairline with or without liposuction of some part of the abdomen.

Type II involves type I and tightening of the abdominal muscles below the umbilicus without its relocation.

Type III also called “umbilical float” procedure involves keeping the umbilicus attached to the skin around it separating the whole abdominal skin and bellybutton from the muscles underneath tightening the abdominal muscles. As the skin is pulled down the umbilicus is relocated one inch lower than its original position and extra skin is removed at the pubic hairline incision.

Buttock Augmentation

Q: Is there a difference between buttock augmentation and buttock lift?

A: There may be some redundancy of the skin, but the buttock augmentation patient is seeking mainly fullness in the region, whereas the buttock lift patient wants to get rid of the redundancy that has occurred due to aging or weight loss and have their butt lifted. A small amount of redundancy can be corrected by increasing fullness with an implant, but a more significant redundancy requires a longer incision.

A: There are three ways to perform buttock augmentation.

  1. With a butt implant.

Buttock implants: Some patients benefit from soft, semi-solid implants instead of or along with fat grafts. They allow us to create pockets that are exactly the right size and location for the implants.

Buttock enhancement is far from a “one size fits all” procedure. The more strategies your plastic surgeon has at his or her disposal, and the more experience the plastic surgeon has with different approaches, the more likely you will be delighted with the outcome of your surgery. Chicago Buttock Augmentation

Thirty years ago, physicians were placing breast implants in the buttock area in order to increase fullness and projection. Unfortunately, this procedure was fraught with complications; some directly related to the implant itself (rupture of silicone, limitations in shape and size of the implant design) and some due to the technique and location of placement. The implants were placed subcutaneously and frequently resulted in an unacceptable shape because of the scar tissue that would form.

As plastic surgery evolved, silicone became available as a soft solid with different shapes and sizes, and liposuction and fat grafting have become popular.

Our aesthetic goals have evolved over time and include the whole area of the lower back, buttock and upper thighs addressing proper projection and fullness. The maximum projection should be at the level of the pubic hair when viewed from the side. The maximum fullness from the back is not in the middle of the buttock when viewed straight back, but in the middle of the buttock when viewed from a 45 degrees angle to the back. The scar has to be inconspicuous. We need to incorporate the buttock with the flank and hip areas in our evaluation, and we have to take into account the trochanteric and thigh areas and decide if they will require special attention.

Until a few years ago we were able to offer only liposuction for this area of the body. Today along with liposuction we can add either an implant or the fat. When fat is used it is harvested through liposuction and then introduced in the buttock area through small cannulas.

In order for the fat to survive, it has to be injected in very minute quantities so that the surrounding tissues can provide nourishment for these cells. If a large volume is injected in any one location, the central part of this imaginary ball of fat and fluid would not be in direct contact with the tissues and not have access to oxygen and nourishment; the fat cells would then die. Because of this limitation, multiple tracts are created in the tissues of the buttock in order for the surgeon to be able to deliver the amount needed. Extreme care with regard to sterility of the procedure must be maintained. Antibiotics are a central component of this procedure. The first limitation for fat injection (grafting) is the amount of fat that any individual patient has available for use. The second relates to the fact that less than 50% of what is aspirated can be used in the augmentation, and the third is that 30-50% of that added volume might be lost over time.

If buttock augmentation is going to be achieved with a silicone implant, the procedure is done through a very specialized incision in the fold between the buttocks. It is designed to prevent separation of the tissue and to prevent accumulation of fluid between the two buttock cheeks that could lead to problems with healing. After this incision is made, a pocket is dissected to the exact size and shape of the implant.

As mentioned earlier, the implants we use today are specific for the buttocks and proper anatomic placement is crucial. Earlier in the learning curve the implants were placed subcutaneously often resulting in the previously mentioned problem. Subsequently, the implants were placed underneath the muscle, but that position suffered two setbacks. The first was that the implants were riding higher than aesthetically desirable and when the surgeon tried to bring the pocket a little lower, pressure on the sciatic nerve was a potential complication.

Today we place the implant in one of two ways. One is within the gluteus muscle itself. The other is underneath the fascia of the muscle. This is the location that I use because it provides me with the flexibility that I need and minimizes the discomfort. The patient is marked before surgery in the standing position with the help of a sizer and dissection adheres strictly to the markings. We inject the area with a long acting pain medication at the onset of the procedure. This seems to help the patient significantly with postoperative pain. A garment is placed at the conclusion of the procedure. Care is simple and within two weeks scar tissue has formed around the implant keeping it in position.

The Brazilian Butt Lift: What’s in a Name?

The Brazilian butt lift is a very popular procedure; in fact it is the fastest growing form of cosmetic surgery today. If you’re considering surgery to round out a backside that appears flat, square or simply too skimpy, it’s critically important to choose a plastic surgeon who is very experienced. Here’s why.

The Brazilian butt lift is not truly a lift. Muscles, fat and skin are not elevated in the same way that facelifts and breast lifts raise sagging tissues. A well-done butt lift appears to raise the buttocks because increased volume enables the skin to re-drape more tightly. There are only two ways to increase the size of the buttocks, and both present challenges for an inexperienced plastic surgeon.

Fat Transfer

Many physicians favor fat injections to add volume to the buttocks. This approach can be successful for a patient who desires a modest increase and has enough fat available from other areas of the body. After harvesting and processing, fat is transplanted in small quantities so surrounding tissues can nourish it. Some fat is lost during harvesting and some does not survive after transplanting—it is wise to plan for at least a 40% loss. A typical 400 cc result for each buttock requires more fat than many patients have available.

Silicone Implants

Silicone implants are a good alternative for many women. The material is a soft semi-solid that will not leak. The implants are safe, do not need to be replaced and feel natural.

The challenge with implants is that some plastic surgeons only use up models up to 300 ccs in size. Larger implants mean wider pockets must be created through relatively small incisions with restricted visual access—a procedure that’s problematic for many surgeons. Dr. Makhlouf developed and patented special instruments for this purpose, tools that expose the proper amount of tissue, flood the area with light and allow for efficient suctioning. These instruments enable Dr. Makhlouf to place implants up to 500 ccs in size. His patients are pleased with their results, happy with both projection and symmetry.

Catchy procedure names abound in the world of cosmetic surgery today, and the Brazilian butt lift is no exception. Not only is the lift aspect of the procedure not strictly accurate, there’s really nothing Brazilian about it. What does “Buttock Augmentation” really mean?

Ok, so you’re confused by all the different terminology in and around Buttock Augmentation. Who isn’t?!!! We’ll simplify the murky waters, here, and define the different procedures. On a positive note, there are only a handful of procedures:

Lower Body Lift:

Goal: Tighten the loose and saggy skin on the butt.

Incision: There will be injections, no incision.

Procedure: Fat is harvested from other areas of the body and injected into the buttocks.

Buttock Augmentation:

Goal: Add volume to the butt that is otherwise flat or not full enough.

Incision: At the intergluteal cleft: the groove between the butt cheeks.

Procedure: A soft and solid implant is inserted into the buttock area.

Result: A full and soft butt that will match the rest of the body.

Labiaplasty

Labiaplasty is a popular procedure that’s sometimes incorrectly referred to as labioplasty. It started as a natural progression over the last decade. Women had been increasingly paying attention to the area of their body covered by a bikini. It started with trimming the hair, then shaving it, and now to permanently removing part or all of it. The advent of the laser and the various reality shows have made it mainstream. There are even articles written about it. In fact, the American Society of Plastic Surgeons (ASPS) notes an uptick in labiaplasty procedures: a 39% spike in 2016, with more than 12,000 procedures.

Background & Concerns

The logical progression was to pay attention to the plumpness of the mons pubis, the rounded mass of fatty tissue that’s over the pubic bones (as per Wikipedia). In recent days, women have started to pay attention to the fullness, or the lack thereof, of their labia majora (outer vaginal lip). Women are also concerned about the size and shape of their labia minora (inner vagina lip). Here’s a great anatomical pictorial identifying the different parts of the female reproductive organ.

The Labiaplasty is a procedure that is directed to the labia minora to address concerns about size, fullness, redundancy and hooding. Some women feel uncomfortable during sexual intercourse, some feel self-conscious during exercise, and in a swimsuit or weight lifting attire. Others also have pain with certain activities like riding a bicycle or horseback riding.

Procedure

Based on the physical examination, various techniques can be used to address the patient’s concerns. Along with the appropriate choice of technique, careful and delicate suturing provides for very satisfying results. The goals are to:

  1. Provide inconspicuous correction.
  2. Minimizes the chance of scar hypersensitivity and painful intercourse.
  3. Heal rapidly.

The procedure can be done in the surgery center or the hospital if there is a request for sedation. But the majority of our patients are comfortable with the procedure done in the office operating room under local anesthesia. It only takes 45 minutes to perform. A very small amount of pain is involved. We do our utmost to provide our patients with a comfortable experience away from any embarrassment.

Post-Op

The post-op care is very minimal and easy to perform. Activity restrictions are:

  • No sexual intercourse for 3 weeks
  • No bicycle riding or horse-back riding for 3 weeks
  • Further, the follow-up appointment schedule is for 5-7 days, 3 weeks, and 2 months.

Liposuction

It’s no surprise that abdominal liposuction is one of the most popular cosmetic surgery procedures in the United States. The tummy and the love handles seem to plague even the most fit and beautiful people and a lot of that is because abdominal fat can be some of the most stubborn and difficult to burn.

Well, the good news is that you don’t necessarily have to turn to a full on tummy tuck to get some good results. In fact, quite often, abdominal liposuction will give you results that will please you, with less risk, less recovery time, and a lower cost.

For women hoping to reduce fat in their tummy area after having a baby or for men who need to hone their physique, abdominal liposuction is the easiest way to go. If you get tumescent abdominal liposuction, you’ll be home and up and around the day of your surgery. You’ll have relatively little soreness and will probably only need Tylenol to eliminate discomfort.

So if you are considering a tummy tuck or an Abdominioplasty, think about getting liposuction first. If the procedure is successful, you will have spent less time and less money on a less risky procedure and you may achieve satisfactory results. If the abdominal liposuction is insufficient, you’ll have completed the first step of your tummy tuck anyway, which is fat removal, and you can move forward with your Abdominoplasty.

Liposuction or Tummy Tuck?

Many patients are unable to decide what the best procedure for them in order to reduce their abdominal girth. The easiest way is to look at what causes the bulge. If it is fat, and the fat is located in the skin then it can be suctioned. If the bulge is from extra skin and stretched muscles then the tummy tuck will take care of the problem. Sometimes we need to do both liposuction and a tummy tuck because multiple factors are involved in creating the bulge and both need to be addressed.

Liposuction or tummy tuck for men?

The mechanism of increase in abdominal girth in men is very different than in women. Men store fat primarily with the internal organs, whereas women store the fat primarily in the skin. The fat present behind the muscle and with the intestinal organs cannot be reached with liposuction. A low carbohydrate diet is a way to reduce that type of fat. Liposuction of the skin fat is possible provided there is enough fat to make a difference.

The only time a tummy tuck is required for a man is if he has lost a significant amount of weight. There will be excess skin removed and the abdominal wall muscles would have been stretched by the excess weight in the skin the man had been carrying around.

Everyone remembers the moment, early in a relationship with a new boyfriend or girlfriend, when you compare scars and tell the stories of how you got them. No one wants to say, “I just got that one after my liposuction surgery.” That’s a reason why one of the fears that patients have about undergoing liposuction is that there might be permanent scarring. After all, liposuction requires an incision in the skin; any time the skin is punctured there is a risk of permanent damage.
